10/9/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. QMHPA informed Ms. Broadnax that this session was going to focus on her mental health. Before QMHPA began the session, QMHPA allowed Ms. Broadnax the opportunity to discuss with QMHPA any current situations that she felt needed to be addressed. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. Intervention: QMHPA assessed Ms. Broadnaxâs current mood of depression focusing on impact on functioning and attempted to resolve it. QMHPA encouraged Ms. Broadnax to speak openly about what she was depressed about. QMHPA and Ms. Broadnax processed the information. QMHPA assisted Ms. Broadnax in identifying and understanding the key unresolved life issues. QMHPA conducted a skills building activity with Ms. Broadnax. QMHPA informed Ms. Broadnax that the purpose of this activity was to assist her with applying conflict-resolution skills to start working towards resolving her issues. QMHPA encouraged and supported Ms. Broadnax in following through with searching for employment, parenting, and social activities as a distraction from her depression. QMHPA reviewed through demonstration and practice with Ms. Broadnax relaxation exercises that supported decreasing her depression and anxiety. Response: Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Broadnax stated she was trying to fight through her depression but found it harder some days. Ms. Broadnax verbally admitted that she had a lot of issues going on in her life all at the same time. Ms. Broadnax and QMHPA engaged in general conversation in reference to the ways she felt she would be able to resolve her issues while decreasing her depressive state. Ms. Broadnax expressed feeling helpless and alone with the struggles of her life but was determined not to let it keep her down for too long. Ms. Broadnax was amenable of the QMHPAâs interventions evident by her communications and interactions.
